PRIZE SURVEY RULES

I. General provisions

1. The Competition is organized by Activlab sp. z oo 02-672 Warsaw, ul. Domaniewska 39a, room 523, NIP: 679-310-46-24, which commissions the competition to Eura7 Sp. z oo, with its registered office in Kraków 31-870, at os. 2 Pułku Lotniczego 47A, a company registered in the District Court for Kraków Śródmieście in Kraków, in the 11th Commercial Division under the KRS number: 0000365066, share capital PLN 20,000.00, NIP: 676-24-29-093, REGON: 0121342416, hereinafter referred to as the Executor
2. These regulations define the rules for conducting the Survey, consisting of completing the survey and awarding prizes (hereinafter referred to as the " Regulations ").
3. The survey is conducted on the website: goo.gl/XWUzuv#AnkietaActivlab

II. Participants

1. The Survey Participant (hereinafter referred to as the " Participant ") may be an adult natural person with full legal capacity and residing in Poland, excluding:

a) Employees and parties to civil law contracts concluded by the Organizer

b) Other people who participated in organizing the Survey

c) The closest persons to the persons mentioned in points a. and b. above, i.e. persons who are such a person's spouse, ascendant, descendant, sibling, relative by marriage in the same line or degree, a person in an adoptive relationship or his/her spouse, as well as a person living in cohabitation.

2. Each Participant may participate in the Survey only once. In the event that one Participant answers from more than one account, only the answers that have been registered in the system as those from the first provided account will be taken into account.
3. Any survey completed incorrectly or incompletely will not be taken into account.
4. Taking part in the Survey is tantamount to accepting its rules as set out in these Regulations.
5. Completing the Survey is voluntary and free of charge.
6. The survey is financed from the Organizer's own funds and is not conducted for profit purposes.

III. Duration of the Survey

The survey is organized from 17/07/2017 until further notice.

IV. Awards

1. The following prizes are provided for 400 people who complete the Survey and meet the other requirements of these Regulations: A set of all I'M FIT Line products worth PLN 32 gross
2. Survey Participants are not entitled to claim payment of the value of prizes in cash.
3. Survey Participants are not entitled to claim an exchange of the awarded prize for another prize.
4. Any claims by Survey Participants for not receiving prizes that were not awarded to them are excluded.
5. The task of the Survey Participants is to answer the questions included in the survey.
6. Receiving the Prize is not dependent on any random event or condition other than those specified in these Regulations.
7. The Survey Participant may not transfer the right to receive the Prize to other persons.
8. The number of Prizes is limited to 400. Once the number of Prizes has been exhausted, no further Prizes will be awarded.

V. Receiving prizes

1. Prizes will be sent to the address provided in the survey.
2. Prizes will be shipped within 30 business days of the announcement of the end of the campaign.

VI. Complaints procedure

1. Any complaints regarding the course of the Survey may be submitted in writing to the Organizer's address no later than 14 days from the end of the Survey.
2. All details will be available on the Organizer's website. The deadline for submitting complaints is determined by the date of the postmark. Written complaints should include the name, surname, exact address of the participant, as well as a detailed description and indication of the cause of any irregularities.

3. Complaints submitted are considered by the Organizer within 14 days of their receipt.
4. The Organizer's decision regarding the complaint is final and binding. The Participant shall be informed of the decision

The Organiser will be notified by letter to the address provided on the application form within 14 days of considering the complaint.

VII. Final Provisions

1. The Organizer reserves the right to verify whether the Winners meet the conditions specified in these Rules, as well as in the provisions of law. For this purpose, the Organizer may request the Participant to make certain declarations, provide certain data, or submit certain documents, including in particular the address of residence or registered office and contact number. Failure to comply with these Rules or the relevant provisions of law, as well as refusal to comply with the above requests, shall result in immediate exclusion of the person concerned and shall entitle the Organizer to refuse to award the Prize and shall result in immediate expiration of any possible right to the Prize or to any other claims against the Organizer.

2. Providing personal data is voluntary. By providing their personal data, the Participant consents to their processing. The administrator of the Participants' personal data is the Organizer. The Participant has the right to access the data and correct it.
3. The organizer is not responsible for:

a) Events preventing the proper conduct of the Survey, which the Administrator was unable to predict or prevent, in particular in the event of accidental events, including force majeure,

b) Failure to deliver/transfer the Prize to the Participant due to failure to provide data by the Participant or incorrect provision of such data.

4. The Organizer shall not be liable for any actions or omissions of Internet service providers, and in particular any failure to perform or improper performance by Participants of the obligations specified in these regulations caused by such action or omission.
5. By registering for and taking part in the Survey, the Participant agrees to the provisions of these Survey Regulations and consents to their content.
6. These Survey regulations are available for inspection at the Organizer's office.
7. The Organizer reserves the right to change the rules of the Survey, including the content of these Survey regulations during the Survey, if this is necessary for the proper and lawful conduct of the Survey. In such a case, the Organizer is obliged to inform the Survey Participants at least 3 days in advance about the content of the changes to the Regulations and the date of their introduction by posting the amended Regulations on the Organizer's website, so that each Participant has the opportunity ^to familiarize themselves with the new provisions of the Regulations and make a decision regarding participation in this Survey or withdrawal from participation in the Survey.
